The goal of this tool is to help you think about your symptoms in terms of frequency and severity, in order to facilitate shared decision making processes with you and your healthcare team.
The tool asks you to indicate whether and how often you have any of 15 symptoms. It also asks you to indicate how often they occur, if you have them.
Each individual symptom has a score between 0-15, which is calculated by multiplying the frequency (0-5) and severity (0-3).
The total score is obtained by adding up all of the individual symptom scores, for a total possible score of 0-225.
